Case Manager
Position Summary
The role involves one-on-one case management (social work), encompassing written correspondence, in-person home visitation meetings with clients, phone calls, maintaining current physical case files, and organizing past client files. Additionally, creating 1-2 client events and assisting with agency fundraisers are also key responsibilities.
Health education and support will be provided, along with referrals to connect clients with medical providers. Each month, focus will be placed on a specific health topic to empower clients to access necessary services.
As needed, recruit volunteers to support programs and events designed for client interaction.
Major Duties and Responsibilities
- Following up and assisting patients with social service and health care navigation
- Using various incentives to increase class attendance
- Navigating patients to other social services
- Providing access to and awareness or resources for patients
- Coordinating with other programs to expand partnerships

Knowledge Required for the Position
Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.
Supervision
Member uses initiative in carrying out recurring assignments following set procedures, independently, The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required
Review
The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.
Guidelines
Written and oral guides provide specific instructions for doing service, Most instructions are easily memorized and require little interpretation
Complexity
The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment
